- A photo ID
- Your insurance card, if you have one
- A list of the medicines you take
- Payment if you are self-pay — cash or card
- Any school, work or DOT form you need completed
In an emergency, call 911. We are an outpatient clinic, not an emergency room.
Before you come in
Should I call first or just walk in?
Either works. Walking in is fine during any open hours and you do not need to phone ahead. Calling first is worth it if you need a specific provider time, want to check whether we take your plan, or want to know how busy we are before you drive over.
How fast will someone answer?
During opening hours a person answers the phone — not a menu. If the line is busy because we are with patients, leave a message with your name and number and we call back the same day.
What language will I be helped in?
Whichever you prefer. Doris Perez, APRN and the front desk both speak Spanish and English. You do not need to bring anyone to translate, and you will not be handed a phone interpreter.

Walk-ins are seen in the order they arrive. If you come right at opening, expect a short wait.
